Screen Charge: $50.00/U per color, per design (based on camera ready artwork)
Artwork Charge: Camera-ready artwork is required. Artwork charge is $50.00/U per hour. Faxed art is not acceptable. Please call customer service for a quotation.
Type Setting Charge: $20.00/U minimum charge (Art department will quote special typestyles or curved copy.)
PMS Match: $27.00/U We will make every effort to match PMS colors when specified, exact color match is not guaranteed.
Print Size: All artwork submitted will be either enlarged or reduced to fit the best imprint size, unless otherwise specified by customer.
Additional Color: Please call Customer Service for quotation.

E-mail Requirement: Please include your company name, phone number, contact name, and your P.O. number on your e-mail.
Artwork Storage Media: 3.5? floppy disk, 100MB zip, 2GB jazz, (DVD-Rom), and CDRom Camera-ready
Artwork: Require high resolution with at least 300 dpi for PhotoShop file (A charge will be incurred for any artwork requiring touch-ups or improvement.)

MAC File Types: (Mac Only) Adobe?s Photoshop 6.0, Illustrator 9.0, Macromedia?s Freehand 9.0, QuarkXpress 4.0, and CorelDraw 8.0
Fonts: All text must be converted to outlines and/or included with the file.

ORDER INFORMATION
Order Acknowledgement: We acknowledge all orders with a fax paper proof unless specified by customer.
Cancellations:
Catalog styles: All orders are considered firm. Cancellations will be accepted only if order has not been printed. Cancellation must be made in writing. Verbal cancellation is not acceptable. 15% restocking fee may apply.
None-catalog styles: customs orders are considered firm after paper proof approval. No cancellation will be accepted. Rush Charges: We will make every attempt to meet your need without incurring additional charges. All customer service for confirmations. Air delivery may be required on deadline shipment order.
Changes on Orders: All changes on orders must be requested in writing before actual production. Verbal order changes are not accepted.
Co-Op Programs: Co-Op pricing is handled individually. Please contact customer service for more details.
Standard Lead Time: 5-7 working days after paper or pre-production proof approval. Blank Goods are shipped within 24 hours.
Repeat Orders: Reference previous invoice number or purchase order number for artwork purposes. $20.00/U per color reset-up fee applied on all repeat orders. All re-orders must be confirmed by fax.
Over/Under Runs: We reserve the right to under or over ship by 3% and bill accordingly. In case of an under run of 3% or less, factory reserves the right not to print the remaining balance.
Less Than Minimum Orders: The minimum order quantity is 50 pcs. per order. A charge of $ 27.00/U will be incurred for orders below minimum requirement. (Call customer service for details.)
Term: 50% deposit is required for all new accounts, first time orders and all custom orders. Balance due upon shipment or credit approved term. Credit card payment is accepted.
SAMPLE INFORMATION Paper Proof: FREE The paper proof must be signed and faxed to factory prior to actual production. Failure in returning the paper proof promptly may delay your order?s delivery date.
Pre-production Proofs: $25.00/U per color Preproduction proof will be sent via over-night courier unless specified by customer. Allow additional 2-3 days for proof delivery.
Custom Design Sample Proofs: Custom design bag is charged based on the difficulty of the design. Artwork charge may applied if a sample or sketch is not submitted. Allow 3-4 weeks for sample delivery. All charges may be waived if an order is received within 6 months. Custom design sample will be marked ?Sample Not For Resale? in order to clear customs. These marks may not be removed.
Blank Samples: Orders must be faxed. All catalog items are available at EQP plus freight charges. Random imprint may be available upon request.
SHIPPING INFORMATION Shipping Methods: CM reserves the right to determine the best shipping method available. All shipments will be shipped via ground service unless specified otherwise by the customer. Air delivery may be required on deadline order.

COD or Drop Shipment: $8.00/U per location, per release, plus freight Drop shipment information must be received prior to production or delay may occur.
Shipping Disclaimer: Due to circumstances beyond our control, CM cannot guarantee continuity of exact shade, color, size, weight, Texture, or construction of finished goods, CM shall not be liable for the inability to ship an order(s) on time. Due to quota, custom, or natural causes, CM will make every effort to meet delivery.
